Community Public Health Teams
Inclusive Action has partnered with AltaMed Health Services and the LA County Department of Public Health to form a community public health team (CPHT) called SELA Florece in Southeast LA. We are proud to have been selected as one of eight CPHTs serving communities identified as the most vulnerable and highest need in LA County for this pilot project. A CPHT is a place-based team that consists of a community based organization, health care partner, and public health staff that is dedicated to connecting with every household in their designated community.

SELA Florece’s mission is to engage, listen, and learn from the community about their public health needs and priorities, coordinate connection to local resources and services that can provide care and support for families, and build a community action plan to drive solutions to priority health issues along the community. We center community voices and lived experiences from the community members we work alongside, knowing they are closest to their community, know what their community needs, and have the solutions.
As part of our work, we have formed a Community Advisory Board (CAB) consisting of eight committed community leaders from Southeast LA that are driving change in their neighborhoods as community messengers, educators, and first responders to public health issues most prominent around them. As we work with the CAB, we are building participants’ capacity and skillsets for them to continue their community leadership in the future.
